Advanced Care Planning
Advanced Care Planning is the process that helps people and their Substitute Decision Maker (SDM) plan for the future.
It is the process of exploring the various possibilities and understanding the options to help you make educated choices for your future health and personal care preferences.
Advanced Care Planning (ACP) is a document that acts as a guide for your SDM.
We will look at each of the various scenarios of being injured, sick, aging or dying, assess the needs for each scenario and the treatment and care you might need.
Your ACP will capture your values, wishes and what is most important to you. It will clearly articulate your instructions to your Substitute Decision Maker (SDM) for treatment and care.
Your wishes can be revised and updated at any time.
Your ‘Will’ can include instructions on how funds are to be used for your care.
This process will happen over 2 - 3 meetings at the end of which you and your SDM will each have a copy of your Advanced Care Plan.
